Remember that the tip is for the server and it should be about 15-20 % of what the meal would have cost you had it not been included. If you are not sure then you might ask the host/hostess for an idea of the cost to figure your tip.

I would still tip 15 to 20 percent. The servers would appreciate it.
It really depends on the service you get (to me, anyway). If you can tell the person isn't trying to meet your satisfaction, then I'd only tip a few dollars (once again.. it really depends for me). But if they're friendly, but not prying into your conversations I'd tip around 7-10 dollars, or even more, if I thought they were really trying to set a good impression without being too nosy.
